NH License Forms
The following license types are available at Fish and Game Headquarters in Concord by application only. Click on the name of the license (below) to download a print-and-mail application form (PDF format).
New! Apprentice Hunting License - Allows persons interested in hunting a chance to do so under the guidance of an experienced hunter without taking Hunter Education. May be purchased once in a lifetime.

Disabled License types must meet certain criteria with application signed off by a physician and are generally only available to residents of NH. The following are print-and-mail applications available for download:
- Complimentary Fishing - Fishing license for the Developmentally Disabled
- Blind Fishing - Fishing License for blind individuals
- Disabled Veteran - Fishing only or Combination Hunting and Fishing (Need Veterans Administration approval)
- Paraplegic - Fishing only or Combination Hunting and Fishing
- Hunt from a Motor Vehicle - For those suffering from paraplegia
- Disabled Crossbow Permit - For use of a crossbow in the archery season (available to both residents and nonresidents of NH).
Miscellaneous Hunting/Trapping Permits available to both residents and nonresidents of New Hampshire; a current NH Hunting License is required (may be purchased at the same time using the license form application):
- Application for Permit to Train Dogs- Allows the training of dogs on wildlife in the off season.*
- Application to Use Dogs to Take Bear - Allows the hunting of bear using dogs.*
- Individual Training and Shooting Permit - Allows sportsperson to possess, release and shoot permitted wildlife for the purpose of training dogs.
- Field Trial Permit - Permit to allow clubs to hold field trial(s) on wildlife.
- Annual Deer Crossbow - Permit to use a crossbow in the Firearm Deer Season.
- Trapping License - Previous Trapping/Trapper Education required.
- Wildlife Control Operator License - A person who is engaged in the practice of taking nuisance wildlife, additional requirements/training courses.
- Leashed Dog Tracker License Application - Required for those providing assistance in tracking dead or wounded big game animals.

Other Miscellaneous Licenses:
- Active Duty Military Licenses - Fishing or Combination Hunting and Fishing License, NH must be home of record.
- Guides License Application - To engage in the business of guiding in the state. Please note: New guide license applicants must contact N.H. Fish and Game's Law Enforcement Division at (603) 271-3127 to request a study packet for testing requirements; this must be done before submitting your application. New in 2011: A N.H. Fish and Game Department review of application is required prior to issuance of guides license.
- Taxidermy Report and Application - Required for those involved in taxidermy (the process of preparing, as in, but not limited to, preserving, stuffing and mounting the skins of wildlife for compensation).
- Summer Camp/Education Facility Permit - Group license for summer camps or educational facilities for students under the age of 20 years.
- Non Resident Honorary - Fishing or Combination Hunting and Fishing License for professional nonresident outdoor writers only.
- Bait Dealer's License Application - Required to take, sell or offer to sell live fish for bait.
- Application to Import Fin Fish for Bait
